Dental Discount Plan FAQ
Terms and Conditions
All plans are effective for one year from the day the premium is paid. Additional fees may be applied for certain treatments – such as gold crowns – but account holders will be notified beforehand by our dental office. Scaling or periodontal deep cleaning are included in preventative treatment, however maintenance cleanings are $75 every 3-4 months as determined by our hygienist. All patient portions are due at the time of service to receive a discount. This is not an insurance plan and cannot be used in conjunction with insurance or any other promotional discount.
